Agreement to Terms

Services Provided

Ume Wellness provides pelvic physical therapy and rehabilitation services. Our services include evaluation, treatment, and management of musculoskeletal conditions, injuries, and physical therapy needs as prescribed by your physician or as determined necessary by our therapists.

Appointments and Cancellations

Scheduling
Appointments can be scheduled by calling our office or through our website. Please arrive on time for your scheduled appointment.

Cancellation Policy
We require at least 24 hours notice for cancellations. Late cancellations or missed appointments will result in a cancellation fee. Repeated no-shows may result in discharge from care.

Payment and Insurance

• Payment is due at the time of service unless other arrangements have been made
• We are an out-of-network provider with insurance but will provide a superbill to give to your insurance company for reimbursement.

Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete health information
• Follow the treatment plan recommended by your therapist
• Inform us of any changes to your health status
• Arrive on time for appointments
• Pay for services as agreed

Treatment Risks

Physical therapy treatment carries inherent risks including muscle soreness, temporary increase in pain, and in rare cases, injury from exercises or manual therapy. You should inform your therapist immediately if you experience unusual symptoms or increased discomfort.

Confidentiality

All patient information is protected under HIPAA regulations. Your health information will not be disclosed without your written authorization except as required by law.
